The facts of this case: The facts of this failure are that : 1.the faulty design of solid rockets booster.

3.lack of proper communication between different levels of NASA .
My conclusions :They should have a lot more communications to find out this incorrect design. Then this tragedy might be avoided. Future environment: In the future, when technique gets much more advanced,and more communication are undergone , this tragedy will probably not gonna happen.
