The trial bore the (barely) hidden name of the company that developed the drug \u2014 <a href=\"https:\/\/www.clinicalkey.com\/#!\/content\/playContent\/1-s2.0-S0140673609620419?returnurl=https:%2F%2Flinkinghub.elsevier.com%2Fretrieve%2Fpii%2FS0140673609620419%3Fshowall%3Dtrue&referrer=https:%2F%2Fpubmed.ncbi.nlm.nih.gov%2F\">SWITCHMRK<\/a>, get it? \u2014 and had a profound impact on how we managed virologically suppressed patients for years.<\/p>\n<p>What did we learn? Namely, that it was risky to switch stable people from their \u201chigh resistance barrier\u201d regimen of lopinavir\/ritonavir plus NRTIs to raltegravir plus NRTIs if they harbored viruses with NRTI resistance. Some of the participants who had a history of treatment failure who switched ended up experiencing virologic rebound with integrase inhibitor resistance, which made the switch to raltegravir not noninferior (sorry for the double negative) to continuing lopinavir\/ritonavir.<\/p>\n<p>The interpretation was that despite the potency and excellent tolerability of raltegravir \u2014 massively better than lopinavir\/ritonavir \u2014 it wasn\u2019t enough to maintain viral suppression reliably unless the NRTIs were also fully active.
